Must be posted in one or more conspicuous places at each insured location.
Must be printed in color.
Size requirement is 11 x 17” paper.
Document is saved with page set up for legal 8.5 x 14” paper and may be enlarged by a copier.
Notice in Spanish is not required by law to be posted but can be offered as a courtesy.
Complete the lower right portion of the posting notice. Fill in the Employer Name, address, policy number and expiration date prior to printing.
