• Both English and Spanish versions are required to be posted and are contained the above attachment.
• Select the Workers’ Compensation Insurance Company from the drop down box prior to printing.
• English version must be printed on Peach 8.5” x 11” paper
• Spanish version must be printed on Tan 8.5” x 11” paper
